Facility Use Request Form

Welcome to our Facility Use Request Form (or FURF as we call it)! For those hoping to utilize CLC or NLPC building space for ministry, community related events, or a private gathering this form is for you.

Most, but not all events need audio visual assistance of some sort.

This allows us to ensure the space is readily available and AC/HEAT scheduled properly.

Our long and round tables each can seat 6-8 people.

Applicant is responsible for setting up and cleaning up after their event. If you agree, please write your first and last name below so signify your agreement.

This includes but is not limited to:
1. PLEASE LEAVE TRASH INSIDE PROVIDED CANS. There will be no extra trash bags available and no dumpsters to dispose of trash bags. The Christian Life Center/New London Presbyterian will handle the removal of all trash. Do not overflow trash cans or put trash next to cans. All trash must fit into cans provided. Removing trash bags will result in them leaking on flooring and carpeted areas.
2. Please leave rooms the way you found it. If more tables and chairs are needed than already furnished you are responsible for setting up and breaking that down.
3. Tables/chairs must be wiped down and put back to their original spot.
4. If using requesting usage of upstairs kitchen must be left the way it was found. You may use the following appliances: microwave, oven for reheating items (no stove top), outlets, fridge, and freezer space. The kitchen is intended for small prepping and not fully preparing meals. Please turn off all appliances used. If counters in the kitchen are used, wipe them clean and ensure that nothing is left on them that wasn’t there before the event. If pots, utensils, etc. were used; they must be washed, dried, and put away. Before using the dishwasher you must read directions posted in kitchen.
5. Sweep or vacuum the floors. Brooms and vacuums are available in upstairs pantry, across from elevator door)
6. Turn off all lights in the rooms that were used at the end of the event.
7. Turn off the fans and lights in the bathrooms at the end of the event.

1. The venues and facilities are used at your own risk.
2. Children should NOT be left unattended at any time and must be supervised by their parent or responsible adult.
3. If staff/personnel on call have any issues with a renter or their guests not following rules, being disrespectful or breaking the law in any way, they are advised to call 911 right away
4. MUSIC can be played during the applicant’s event. However, music should be appropriate for all ages (even if there are no children present at the applicant’s event) and should be played at a reasonable volume.
5. NO animals are permitted in our indoor venues. Excluding service animals
6. SMOKING is NOT permitted inside or outside buildings.
7. ABSOLUTLEY NO ALCOHOLIC BEVERAGES ARE PERMITTED ON ANY OF THE PREMISES AT ANY TIME.
8. If spending the night, boys and girls MUST sleep in separate rooms.
9. In case of inclement weather, we follow the Avon Grove School District closing. If an in-church activity or an emergency arises (such as a funeral), your event may be relocated or cancelled.
10. Person completing this form is responsible for any damages to any property used.
